Davis, North Annex donated records of institutions and departments patient indexes and registers, 1880-1952 in May 1996.  The material was processed by Steve Daily.  The collection was reprocessed on October 31, 2002 by Kevin Abing.  Additional material was added to the collection in 2008 and was processed by L. Chasco.
